Does anyone know if there are any additions to some of the features?
Example:
In the rules portion it just has turning as the only option. Has anyone already done the work to add anything else.

Monsters: Has anyone added any monsters from any other sources?

If you join the yahoo group, which is http://groups.yahoo.com/group/dmsfamiliar/, you'll find a whole bunch of stuff under files. There's a community that is fairly active in getting stuff (rules, monsters, feats, spells, skills, etc) added. They're also quite responsive with answers to questions.

What Setanta said. The Yahoo group is the place to look for extra info to import into the program. I think there are 200+ rules that have been typed in so far.

DM's Familiar supports importing and exporting for all of it's database items (Rules, Spells, Skills, Monsters, PC/NPCs, Feats, Weapons - am I forgetting anything?) which makes sharing information between users very easy.

Oh yeah, you can Import/Export the Codex Tree as well. The Codex Tree is an outline/rich-text tool. So you can write an entire adventure in the Codex Tree, cross-link it with Monsters/NPCs/Spells/etc. from the databases and then export the whole she-bang to share with others.
